Mg sulfate heptahydrate
Indications/Uses
Control & avoid convulsions in pre-eclampsia & eclampsia.
Dosage/Direction for Use
IV Loading dose: 4-6 g IV followed by 1 g IV until 24 hr post-partum. IM Loading dose: 4 g IV + 10 g IM can be added 2-4 g IV. Can be added 250 mg Na amobarbital then 5 g Mg sulfate 4 hrly until 24 hr post-partum.
Contraindications
Non-conforming patients to +ve patella reflex; diuresis >100 mL/4 hr; no sign of resp depression; respiration >16x/min. Heart block. Renal failure (CrCl <20 mL/min). Not to be used during pregnancy 2 hr prior to delivery.
Special Precautions
May precipitate an acute myasthenic crisis. Renal impairment. Lactation.
Adverse Reactions
Painful IM inj; hypermagnesemia (nausea, vomiting, flushing, hypotension, muscle weakness, muscle paralysis, blurred or double vision, CNS depression & loss of reflexes), severe hypermagnesemia (resp depression/paralysis, renal failure, coma, cardiac arrhythmia & arrest); hypocalcemia w/ tetany secondary to hypermagnesemia.
Drug Interactions
Ritodrine HCl. Heart block w/ cardiac glycosides. Enhanced CNS depressant effect w/ CNS depressants. Excessive neuromuscular blockade w/ neuromuscular blocking agents. Exaggerated hypotensive response w/ nifedipine. Ca sulfate may precipitate when Ca salt are mixed in the same IV soln. Incompatibility w/ alkali carbonates, bicarbonates & soluble phosphates.
